Cancer support workers offer a “one call” for patients who are currently receiving cancer care. The team support interventions such as Personalised Care and Support Planning (PCSP) and Cancer Care Reviews and will liaise with secondary and tertiary care and the voluntary sector, working closely with PCN Cancer Champions where appropriate.
