The Well Up North App provides patients in Northumberland easy access to the NHS personalised care services available at their GP practices including Social Prescribing, Social Prescribing for Dementia, Health and Wellbeing Coaching and Cancer Support.

The app is free, easy to use and will allow you to contact local services, request, book and manage appointments as well as a platform for video calling.

The app is available for patients at the following GP practices:

Alnwick Medical Group (Alnwick, Seahouses, Embleton and Longhoughton)
Belford Medical Practice (Belford and Seahouses)
Coquet Medical Group (Amble and Broomhill)
Gas House Lane Surgery (Morpeth)
Greystoke Surgery (Morpeth)
Northumberland Health (Widdrington and Felton)
Union Brae and Norham (Berwick upon Tweed and Norham)

Well Up North are aware that peoples physical and mental health and wellbeing is influenced by a range of factors, including their housing, finances, personal relationships, and existing health conditions. Not all the factors that affect someones health and wellbeing can be resolved through conventional health services. This app aims to ensure they receive the right support from the right professional, at the right time to support patients, by providing a person-centred conversation based around asking, "what matters to you?"

What is Social Prescribing?
Social Prescribing provides practical support to patients with their social and emotional needs affecting their health and wellbeing and increase peoples active involvement with their local community, by connecting them to non-medical community-based activities, groups and services that meet practical, social, and emotional needs.

Contact can be through face to face in practice, home visits, telephone and video calling and is offered for a maximum of 12 sessions.

What is Social Prescribing for Dementia?
Social Prescribing for Dementia provides support to patients who are beginning to experience memory loss or mild cognitive impairment or have a diagnosis of Alzheimers as well as their care givers, families, and close friends. The service provides support around loneliness and isolation, fear and anxiety and help accessing practical support services, local groups and support around making informed choices around Power of Attorney, applying for Attendance Allowance and Blue Badge.

Initial contact is made by telephone and followed up with a home visit where a plan is developed around what help is needed and this support is ongoing for as long as is required.

What is Health and Wellbeing Coaching?
Health and Wellbeing Coaching is a person-centred approach, working together with you around what is most important to you. Coaches will support you to improve and manage your own health and wellbeing.
Your coach will work with you on a 1 to 1 basis to explore possible lifestyle changes including diet, physical activity, sleep or managing existing conditions.
Together you will set realistic goals, discuss motivation and your coach will be there to support you on your journey to better health and wellbeing until you are confident to continue on your own.
What is Cancer Support?
Cancer support workers offer a “one call” for patients who are currently receiving cancer care. Support is provided directly to patients and their carers and ensures patients have the right support at the right time to understand their diagnosis, coordinating the pathway for patients with cancer and what the next steps are to help them. The team support interventions such as Personalised Care and Support Planning (PCSP) and Cancer Care Reviews and will liaise with secondary and tertiary care and the voluntary sector, working closely with PCN Cancer Champions where appropriate.
